Is there a chance to get a signal ( switch item) on OH 3.4.1 raspberry 4 , when my iPhone is ringing or it receives a call. The iCloud just supports iPhone location.

I don‘t think so. Siri shortcus and IFTTT do not support incoming phone calls either. Missing API is probably a security feature.
Maybe bluetooth could be an idea? You need to find a device which behaves like earbuds and has an interface.
