There is a bug in Habmin about the UI display of associations. You should be able to see them in PaperUI, but still best to do zwave configuration in Habmin…

Depending on the type of de… vice, associations display as empty or a blank with an X...
PaperUI displays them correctly for me. I see this with the dev version of the binding, but IIRC, others have reported this with the 2.3 release version too.

I have a few things that every ones in a while lose their lifeline.
For a few days everything works well, but suddenly (for example) clicking on my Node Octan will not make the music start. I go check the log and it says “[me.event.ThingUpdatedEvent] - Thing ‘zwave:device:mydevice:nodeXX’ has been updated.”, but there will be no log entry for Item change.

It sounds like you may have switched zwave bindings a few times. Did you delete all of your zwave Things each time (different than exclude)?
